The on line registration process will guide you through registering and
paying for the conference using a series of interactive forms. When you
have completed your registration, you will receive an emailed receipt.
Before you begin, you should have the following information available:
۰ A email address to which the confirmation
and receipt can be sent
۰ A password so you may make changes after
you have registered.
۰ Your choice of the focus clinics and
workshop you want to attend and an alternate.NOTE: Each focus
clinic and workshop has a limited enrollment. If your first choice
is full, you will need to pick another focus clinic/workshop.
۰ Your choice of smoking or non smoking hotel
room.
۰ Your payment method: i.e. check, purchase
order, credit card. We accept all major cards.
Cancellations must be received in writing or be processed on line
through the registration site. (You will need the password you created
when you registered.) Requests post marked or dated on line prior to
March 24, 2008 will receive a full refund less a $150 processing fee.
There will be no refunds after March 24, 2008. No refunds or credits
will be granted for late arrivals, early departures, or unattended
sessions. You may send a substitute.
